All patients registered with Staines Health Group are allocated a named accountable GP. Patients are advised of their named GP when they register. Existing patients have been advised via messages on repeat prescriptions, text messages, emails or by practitioners themselves.
If you do not know who us your named GP, please feel free to contact us.
Having a named GP does not change your right to ask to see a GP of your choice.
Patients Aged 75 Or Over
This GP will take lead responsibility for ensuring that all appropriate services are delivered and where required will:
- Work with relevant associated health & social care professionals to deliver a multi-disciplinary care package that meets the patient’s needs.
- Ensure that the physical and psychological needs of the patient are recognised and responded to by the relevant clinician in the practice.
- Ensure you have availability to a health check should you wish to have one.
In this practice, you are still able to see a GP of your choice, especially if your named GP is absent.
